Join us for the Annual Foundation Golf Classic!
Proceeds of the tournament support the work of the Great River Health Foundation, serving patients and families across Southeast Iowa, Western Illinois, and Northern Missouri. Fort Madison tournament date is June 21, 2024. Burlington tournament date is September 12, 2024.

Registration Instructions:
$500 per foursome in the tournament of your choice. $1,200 for a group of 3 foursomes in the tournaments of your choosing. Each tournament will feature 18 holes of golf. Team registration includes: Greens fee for 18 holes, golf cart, lunch, snacks and drinks.
Registration deadline for Fort Madison Tournament is June 11, 2024.
Registration deadline for Burlington Tournament is September 3, 2024.
