Social & Community Context

You Matter!

Health starts in our families, neighborhoods, schools and jobs.

Did you know that your quality of life started when you were born and is impacted by where you live, learn, work and play? Your social and community circumstances directly affect overall health and well-being. This includes relationships with family, friends, co-workers and community, as well as living in unsafe neighborhoods, experiencing discrimination and not being able to afford life’s necessities. Interventions to get social and community support are critical for improving health and well-being.

The Social Determinants of Health information is from Healthy People 2030, U.S. Department of Health and Human Services, Office of Disease Prevention and Health Promotion. For more information, visit: https://health.gov/healthypeople/objectives-and-data/social-determinants-health
